It's a automation problem.
To solve the problem:

-Run VB6 and open a new standard project
-Unload all Add-ins(from Add-ins manager)
-Right-click on a toolbar and select the Customize... menu.
-For each toolbar click the Reset...
-Reload the add-ins.
