Just check the project code though. The line:
Set Dbs = OpenDatabase(App.Path & "Contacts.mdb")
Needs another "\" before "Contacts" so it looks like

Set Dbs = OpenDatabase(App.Path & "\Contacts.mdb")

Try it out, (Download Contacts.mdb) and then customize the code to open your own database with your own fields
