Multiple Criteria for opening a form

I am using VBA and up until now I have only wanted to sort the form by 1 item. Now, I want to sort by 3 different items at once, but I can't see a way to do this without a ton of coding and a lot of researching on my part.

Anyone know of an easier way to do this? Or, if you don't could you give me a push in the right direction?
