Make sure you have the WinForm folder under the VBWizards directory. This should have two subfolders (Scripts and Templates - that each have a 1033 folder). In the 1033 folders you should have a Default.js file for scripts and Form.vb in the Templates.

    there are two default.js file

    Winform\Scripts\1033
    Dataset\Scripts\1033

I have been able to insert new forms in the past. This error message just began. I also created a new project and tried to insert a new form and still get the same error msg...any ideas?

Power being cut could do it! You must be in the NorthEast too! :-)

Sounds like your file structure got mixed up a bit. I would also recommend a reinstall - even though it takes forever!!!
