You would have to do some shell programming in VB. That’s not what VB is supposed to serve as, but it is possible. I remember there was a book about shell programming from O'reilly. I can't remember if it uses exactly the same example as you have on your picture, but I remember that it had an example that could change the pop up menu in Explorer ( like WinZip does, if you got WinZip )....
