Hi -

I don't get to work with forms too often but here's my question:

I need to have 4 checkboxes on a HTML form. The last checkbox should also include a text box to be able to type in it. How do I do this?

For Example:

(checkbox) Option 1
(checkbox) Option 2
(checkbox) Option 3
(checkbox) Option 4, please explain: _________________

The please explain should be a text box.
