Rerouting/Hooking/Subclassing Eliminating

How can I hook the message routed to this form (see file) which is called when Outlook Wants to locate Contacts and insert my own Form (As in My VB Proggie)

If I open a new message in Outlook
and I wish to add a user , I click on the TO, BCC, or CC buttons and i get this window.

I want to override it, with my program.
