I don't think that posting screen shots is going to aid anyone in helping you. What method does your voice modem use to output audio?.... MSComm is strictly ANSI and Binary compatible, so I don't think you can live stream audio with the Output property. I'm not that familiar with TAPI, or its abilities, but is this what you're using?

Is this going through a sound card or through the modem's voice speaker phone???

I think the Realtek device is your PC sound card -- not your modem. What is it that you are trying to do? If dialing is the goal, then the modem is the one to use. If your modem is not a voice modem, then it will not have an associated sound driver.
