Download EZTwain.dll Classic from Dosadi.com. It appears to be the best free option for interfacing with a TWAIN scanner on Vista. There are several sample projects out there. Also, in the Codebank there is guide for using WIA (Windows Image Aquisition). However, this doesn't work in Vista. MS has replaced WIA. Hope this gets you going in the right direction.
