Help needed in database!

Can you help me guys?

I'm currently using VB6 and currently studying about the database...

Now, I've created a fill-up form where you can add, edit, and delete profiles.
The form contains a surname, first name, and a middle name..

The problem is, I don't know how to create a search button. Where if you click on the search button, another form will show up. The second form contains a list box, a textbox where you will input his/her surname, a command button, and three textboxes where you will see his/her record.

Once you type in the name of the person, his/her name will show up on the list box. And when you click that name on the list box, his/her record will show up on the 3 textboxes..

Guys, help me with this one. Thanks...

PS: Could you make it simple? Ahaha...
