Password Protecting Forms

When you add a form to a project, choose the LOG-IN Dialog for a sample. This can get you started

Project --> Add Form then click Log-In Dialog

Does anyone know of any good code that I could use to Password Protect a form so that anyone not knowing the password could not use it or enter it?
