Message Tracking Not Working Properly
I sent a PM to a user on the 24th of April. When I look at Message Tracking the message does not show up in either Sent Messages: Unread by Recipient or Sent Messages: Read by Recipient. Note that the Show Messages From: value was more than enough for it to have shown up. I can see the message if I swith to the Sent Items view.

I just ran through a few tests and everything seems to be working.
Are you sure you didn't uncheck the Request Read Receipt box? If it isn't checked, then the message is not tracked.

99% positive. It could have been by accident but I've never intentionally used any of those checkboxes.

Well it doesn't show up in either Sent Messages: Unread by Recipient or Sent Messages: Read by Recipient just like the PM that started this, so that is at least one way it could have happened.

Which, since I 'denied receipt', that kind of makes sense. Right?

Wouldn't that be like informing you that I don't want to inform you that I just did something with your message?
Thus this would completely nullify the entire concept of 'deny receipt'

Although this is nullified to some extent anyway, because I assume the message is in the not read section until the receipt is denied at which point it is dropped out. As such, if it is listed, then you knowit was sent. If it isn't listed, then it was sent and receipt was denied.
To summarize -- I'm not sure that there is anything that can be changed that wouldn't break the functionality of something else.
