error accessing the system registry
I have an application running on Windows 10 and when I try to load the VB6 project I get this error, "Error accessing the system registry". The user account has admin privileges on the PC, though the Windows installation hasn't been updated in 5 years. I know I've run the VB IDE there before (I modified the code 5 years ago), so what else can be wrong?

Do you start VB6.exe elevated?

What do you mean by "elevated"? There isn't a right-click menu option for "run as administrator", if that's what you mean.

You should be getting a UAC prompt when you start it, you know the one that asks if you want to allow the software to make changes to your pc. If you are not getting that prompt at start up then either you are not running it with the proper access or you have disabled the UAC options.
When you say there is no run as admin what are clicking on? It should be there on the VB shortcut, it may not on a VBP file.
Normally you would just go to the properties of the vb6.exe and tick the little box to run as administrator so it always tries to run as admin even when starting it by clicking a vbp.
