    That's a pretty big question.
    Start a project, pick Active-X control from the type of projects available.
    Create the interface as normal using the normal listboxes, text boxes etc (don't put any code in yet), and then I would advise using the ActiveX Control Interface Wizard add in - use the Add In manager if you can't see it on your add in menu.

    This will set up most of the common events, methods etc - and you will be able to see from that how to amend them to suit your own requirements.
