The "sick" smilie doesn't look bad.
As a 'heads up', not all the smilies need to be round circles with faces. They can be other tiny iconized graphics as well.
Additionally, the format will be .gif. bmps are easy to convert to gifs. Being .gifs, animations work too. There are a number of animated gif editors available as shareware / freeware.
