Forms on intranet

I set up a form for help submission on my intranet. When I test it, it is giving me an error:

I'm guessing with the info you posted, but try reinstalling Frontpage extensions on the server

what does the log file say?

It was that the smtp address was not correct.
Fixed it, works fine now.
