Create table of content in micrsoft word?

Hi all can anyone please help me and let me know step by step how i can create my table of content as i have this below and need make it as index. Ih vae selceted them and went to insert then Reference adn then index and table but not working it just give like this

I dont know why that thanks all.

1) set a (new) style for the TOC headings
2) write your text
3) highlight the heading lines and use the style you created
4) Back to the beginning - Menu>Insert>References - index n tables

Follow tab to TOC and pick your style ...
