Need a good ICS software or Proxy Server software (Wingate Upgrade)

I have an older version of Wingate Server, but I am looking to upgrade my proxy server that I use to share a broadband internet connection in an office building.

These features are a must:

Username and password access security
Permissions based on IP or MAC address
Limit the number of ports or connections that can be opened by any specific client on network
Limit or restrict what websites or ip's can be viewed or utilized
Limit the amount of bandwidth each client can consume (bandwidth throttling)
Limit the amount of bandwidth each client can consume
