not getting input from forms for reports.

hi

i am getting the reports but when i need a report from a form.
i.e i need to take input from the forms.
how can i do that .

when i wrote like this in the command1:

SELECT [KEY], [AREA OFF], [OARF VALUE], [ACT P VALUE],
[ACT A VALUE], [ACT GLASS VAL], [ACT BOARD VAL],
[ACT BAR VAL], [ACT MESH VAL], [INST VALUE]
FROM MAIN
WHERE CLIENT = '"& COMBO1 &"'

empty fields are comming without any data. from my database.

please anybody solve my problem.
