open form from report template

hi
i have a empty template of report which having 4 rectangles.

i want to change height and width of rectangle at runtime by clicking on the report

when ever i clicked on ther report it should open a form .

their i will enter value

when i say ok it should affect back side i.e rectangels

how can i do this ?

can any one help me?

What kind of a template is it? VB? CR?
