mySQL Databases?

Does anybody know how to:

1.) Connect to a mySQL db
2.) Make a mySQL db
3.) Make entries in a mySQL db

i.e.

my_db
|_site
   |_http://www.yahoo.com
   |_http://www.vbforums.com
   |_http://www.barnesandnoble.com

or

insert assosiated entries

i.e.

user, pass, email

or

name, address, phone
