For a form, how do I set it to where it would have no icon in its caption?

For a form, how do I add a custom-made box to its caption?

And how do I work with resources, particulary icons? I know how to load one, but how do I change its constant name and put it into my application? I would like a detailed explanation, or at least a link to a detailed explanation, if possible. Tnx.
