Create image from text and other images

with reference to above image i want to know is it possible with PHP to keep same above image format already built in.

And in our PHP form we just enter the text fields of users and upload user image and when click on save button we get an image ready with new user name, address and photo.

we want to built this card digitally from our website.
User will go to our website after login. And fill the form contain his name, address, upload his image etc. and When he submit the form. then his card appear in a JPG or PNG format. same as the above image
