I need to make a form to collect name and job of people as a sing and to submit auto this to a list of sings whit a counter.
Sorry but I don't understand the question.
There is list of songs and people can vote by typing name and job. After they click submit I want to add numbers beside songs and create a list by name and job they enter. Better now?
Let me try rephrasing. You want to display a "voting" form for each song and users would be able to cast their vote by entering their name and job. After they cast their votes, the vote count should be displayed along withe song listing. And also, you want to list out the voters name and job beneath it. Is that right ?

Check out the wordpress forums site... there are plugins and add ons that do that already. I think there is one that's even designed for songs and such.
