Checkbox in a toolbar?

I'd like to put a label and checkbox side by side in a custom Excel toolbar, that looks something like:

Code:
+-------------------------------------+
options
+-----------------|           |-------+

opt1   x
opt2   x
opt3   x
+-----------+
(it was quite a challenge to do this -- I hope it comes out OK). How can this sort of thing be accomplished?

Thanks!!!

What about using a drop down combo box?

Use Type:=msoControlDropDown or Type:=msoControlComboBox
