In need of major help!

I hope its visible enough, But I am a beginner programmer in school right now. I need to program this in Visual Basic for school and its due tomorrow, I'm a slacker I know. Its a pizza ordering form. I have very little programming experience so any help with this would be greatly appreciated!

In addition, there are different versions of Visual Basic with some pretty substantial differences in snytax. What version would you happen to be using?

I get $100 an hour to code when I'm not at work. Payment is expected at time of dilivery on the EXE file that you want. How will you pay. I expect at least 2 hours of work. 1/2 up front please.
