The end of evolution
Do advances in medical science mean that survival of the fittest no longer really applies, as nowadays we can keep alive so many people in inhospitable conditions at least long enough to reproduce?
Does this mean that specimens like visualAd are as evolved as humanity is going to get?

Three—no, four—thoughts.
1. Define "inhospitable conditions".
2. Since evolution only applies to genetically determined traits, you must be referring to the reproduction of people with genetic diseases that significantly affect their quality of life. Since these are, by and large, recessive—or at least, very uncommon—it is not necessarily a 'bad' thing (in an evolutionary sense) for people afflicted by such disorders to reproduce. This minimises the effect of what you I think you are describing.
3. Consider that social standards and stigmas play such a large role in determining natural selection, in the modern world, that the role of genetic disorders is most likely insignificant.
4. visualAd should not be used as an exemplar of any kind.

Evolution won't stop. Random mutations are always creating new genes and entering the gene pool. As long as the new genes don't severly inhibit a person's ability to reproduce, it'll stick around.
In fact, evolution might be causing us to be less physically fit than before, even though we may be larger than our ancestors because of the better diet.
Example? Freaks like me who need glasses to see past 3 feet would have never survived in the hunting and gathering times long enough to find a wife.

Do you think that we couldn't think for ourselves 500 years ago?
Of course we control our own evolution. It's simply the case that we have different criteria for selection now than we did hundreds or thousands of years ago.
On the other hand, an example of unnatural selection is plant or dog breeding.

As long as women are choosy, there will be natural selection. The criteria might change, but as long as some selective reproduction is occurring, then all's well.

I tend to go for women who don't have a pulse. That way, the cost of shopping is dramatically reduced.

You would have to be pretty hard up for a date to have sex with someone on life-support.
And evolution has not stopped. It is still going strong adapting people to the environment they live in. Why do you think Kenyans always win distance events? They are adapted to high-altitude and process oxygen better. Whereas if i went to africa to join a tribe i would probably die of sun poisoning.
