How long would it take someone proficient in VB to create this?

When proficient in VB how long would it take, in hours, to create a program as follows.

One menu form to select one of six other forms.
Four text boxes, six option buttons, and three command buttons on each form.
One of the command buttons sends, via winsock, the data in the text boxes and a "flag" as to which option button was pressed to an E-mail address. The other two simple clears the text boxes or exits.
The data in the text boxes and the option button "flag" needs to be appended to one of six Excel spreadsheets.
