I think a bit more information is in order. Assume that we don't know anything about what you're doing, because we don't. Your post should be concise, i.e. brief AND clear, not just brief.
Ah... I didn't see the image. I use Firefox and have it set to not show images from other servers to cut down on advertising. You can attach an image to your post directly by using the Manage Attachments button on the Advanced page.
Anyway, are you saying that you want it to look DOS-like as it is in the image or are you saying that you want essentially the same layout but with a modern Windows XP look? Again, don't assume that we know what you're thinking.
I'm confused. So you want a DOS like application, yet you want it to work via Touch Screen? That's very akward. Are you just trying to re-create the elements in a Form or do you actually want this based in DOS?

I'm sorry but I still have no idea what you're asking. Do you want to create an application that looks exactly like that (minus some text changes and such), will work on all versions of windows and be usable via a touch screen?
If that's the case, you're going to have quite a difficult time doing so. Not only is that user interface ... old but it's just not modern. It's not something many people today are used to and it's kind of counter-intuitive especially in today's point and click world.
Most applications that had a GUI like that required use of the TAB, ENTER or Arrow keys to move from field to field and you couldn't use a mouse so you won't be able to get a UI like that to work in a touch screen environment without some major hacking (which, again, is not intuitive).
