I'm sure this way to easy to be wasting everyone's time for, but I don't program in Word or Outlook.. so..

I want to create a form (of some sort) that uses radio buttons to allow a user to answer a couple of yes or no questions. This form would be inserted into a word document as the last page. Once the friendly user has answered my questions, they would click on a button that would send this form back to me via email (in Outlook). Something like an email address hyperlink that copies the contents of the page to the email body. The user would then click send button and I'd have my feedback.

Can someone please point me in the right direction to starting this?
This is an extra task that I decided to take on in hopes that I'll get paid more money. Naturally, I'm hoping someone else has already done the grunt work and I can reap the benefits... eh?
