    Welcome to the forums icewater.

    You have posted an impossible question to answer without securing a lot more detail.

    What kind of report?
    What are you going to use to create the report? (Crystal Reports? A text file report? ReportWriter? MS Access? Something completely different?)
    How, and from where, are you going to call the report?
    Where will the data for the report come from? Database table? Text file? Something else?

    a simple hint, if you have a database just link your report (wheter it's crystal report or datareport) then add your fields
