I want to have a form in my ms word document in which when i'm going to print it, i'm going to use a ready made form as my paper source.

The thing to be done is the only thing that could be printed is the values that would be inputted on the form. The form on the documents should only serve as a layout or background.

By the way, when i tried scan the form and make it as my background, the image is too large..
